Obtain Repeat Prescriptions?

Repeat prescriptions can only be ordered on a repeat request form or by completing one of our order forms which are kept at reception. We offer an online prescription ordering service (please phone or ask at reception for details). You can also order via fax or post but, please, not by telephone. Please allow 48 working hours before collection and make allowances on weekends and bank holidays.

Where possible, please give the exact drug names when ordering. Ordering medication is your responsibility.

Obtain Sickness Certificates?

A sickness certificate completed and signed by the doctor is only required when a period of continuous absence from work due to illness is of seven days' duration or longer.
