Obtain Repeat Prescriptions?

Repeat prescriptions can only be ordered on a repeat request form or by completing one of our order forms which are kept on reception. We do not accept telephone requests but we are happy to accept requests via fax or post.

Please allow 48 working hours before collection and make allowances on weekends and bank holidays.

Where possible, please give the exact drug names when ordering. Ordering medication is your responsibility.

How Do I....
Obtain Sickness Certificates?

A sickness certificate completed and signed by the doctor is only required when a period of continuous absence from work due to illness is of seven days' duration or longer.
